Advance care team

TotsnTeens is recognized by NCQA as a Level 3 Patient-Centered Medical Home.  Tots n Teens Pediatrics is also recognized as part of the Blue Quality Physician Program by BlueCross BlueShield of North Carolina.

We are fortunate to have an Advanced Care Team led by Dr.Gopan.  Dr.Gopan is assisted by Andrea B and Jacklyn S.

The advanced Care Team provides the following:

• Continuity of Patient Care
• Coordination with Patients to help them with
  
  • navigation of healthcare system
  • referrals to specialists
  • referrals to hospitals 
• Patient Services
• Community based services :  Mental and behavioral counseling services through New Leaf Behavioral Health
• Build relationships with the other doctors and hospitals to refer patients and follow-up on the care they are providing our families.
